Key Review Excerpts

In less than two weeks she ended up with a severe 2 inch necrotic pressure wound on the back of the left heel as a result of St. Charles' staff redressing and re-applying her surgical splint with no attention to protecting the heel.

Memory care family member · 2025☆☆☆☆

She is always clothed, they make her to get out of her room and go eat in the dining room, they call anytime something changes/happens, the rooms are clean* and the people are happy to help with anything.

Long-term resident's family · 2023★★★★★

This place was a nightmare she would call us crying because no one would answer her call light we would call & ask them to check on her. Very under staffed with no compassion.

4 complaint-triggered

This facility has faced five complaint-triggered inspections where families filed reports about serious issues including pressure ulcer care, accident prevention, care planning, and infection control. The most recurring problems involve fire safety systems, environmental conditions, and care planning deficiencies. While all deficiencies show correction dates, the pattern of ongoing complaints and repeated issues with sprinkler systems and fire drills across multiple surveys raises concerns about sustained quality improvements.
