What this means for your family

While the facility is physically beautiful and offers excellent physical therapy, the recurring reports of neglect and slow response times are critical red flags. If you choose this facility, I strongly recommend visiting at irregular hours, including weekends, and maintaining a very active, daily presence to ensure your loved one's basic needs are met.

Key Review Excerpts

Not good at following up with families (answering their phone calls, caring to receive pertinent medical information from the family) and providing patients with accurate dosing and administration of medication. This had dangerous consequences, put the patient in distress, and led them to the ICU.

Family member · 2024☆☆☆☆

I believe my father's health declined rapidly to the point of having to let him go because of long term lack of care. He was unable to feed himself and they let him lay there for months barely eating and barely drinking.

Long-term resident's family · 2024☆☆☆☆

The physical therapy staff are awesome. The social services team are not. My mother was here to get rehab from a fracture, she spent about 3 weeks here and had her phone taken away, sat at the nurses station for punishment.

40 complaint-triggered
$99,184 in fines

This facility has a concerning pattern with 73 deficiencies across recent surveys, including 40 triggered by family complaints about abuse reporting failures, accident hazards, and pain management. Recurring problems involve care planning and assessments, nutrition and dietary services, and infection control, with issues persisting across multiple surveys from 2023-2024. While the facility reports correcting violations, the high volume of complaint-driven deficiencies suggests ongoing quality concerns families should carefully consider.
