What this means for your family

This facility is highly regarded for its physical therapy and short-term rehab outcomes, making it a strong candidate for post-surgical recovery. However, families must be vigilant regarding medication management and communication; we strongly advise setting up a clear, documented plan for updates and checking in frequently during weekends to ensure your loved one's needs are being met.

Key Review Excerpts

The physical therapists were very good and understanding, and after arriving and not being able to lift my right leg off of the bed, they worked with me to regain my strength and walking ability.

Rehab patient · 2022★★★★★

They never notify you on there care even when you ask. They never answer the phone on the weekends so you can never contact your loved one. They accidentally gave my husband a Medication that is given to those with schizophrenia and never notified me of the medication error.

Long-term resident's family · 2021☆☆☆☆

I supplied her meds, incontinence care supplies, oxygen, and nebulizer. I left explicit instructions for the administration of the meds and nebulizer treatments. I was assured time after time that they were on top of it and that their staff was highly trained and competent.

This facility has concerning abuse and neglect violations from family complaints in 2023, including failure to protect residents from abuse and mishandling of resident belongings. The most recurring issues involve fire safety systems, emergency preparedness, and nutrition/dietary services. While all deficiencies show correction dates, the pattern of serious safety violations alongside complaint-triggered abuse concerns suggests families should carefully evaluate this facility's resident protection practices.
