Key Review Excerpts

I had to put my dad some place that he would be able to meet his needs and also someplace that understood what being a veteran means and this place meets every thing and more! Everyone cares, the staff is absolutely AMAZING.

Veteran's child · 2024★★★★★

The Department of Health is investigating the facility for repeatedly breaching patient safety, dignity, and care policies. Furthermore, facility leadership, such as the Director of Nursing (DON), commandant, and various upper management members, were revealed by the department for hiding and providing false information about a preventable incident that led to a resident's accidental death.

Concerned observer · 2024☆☆☆☆

Families have filed complaints triggering inspections, with the most serious involving abuse prevention and accident hazards that prompted family reports. The facility shows recurring problems with care planning, safety hazards, and resident rights issues across multiple surveys. While all deficiencies show correction dates, the pattern of complaints and repeated safety concerns in areas like accident prevention suggests families should thoroughly investigate current conditions before visiting.
