Key Review Excerpts

The staff doesn't care about your needs and this includes the nurses. The food is terrible and the lowest quality imaginable. I wasn't washed daily nor were my teeth brushed.

Rehab patient · 2025☆☆☆☆

My mother almost died in this facility after not being fed. She's type 2 diabetes and is on insulin. She became unresponsive and then they overcompensated giving her too much insulin.

Long-term resident's family · 2023☆☆☆☆

I refer to this facility as a hell hole. Rude non caring staff. How they pass any Medicare inspections I can't imagine. Cleanliness does not exist in this facility.

13deficiencies
Above state avg (10.1)
3 complaint-triggered

This facility has a significant pattern of deficiencies across multiple surveys, with families filing complaint reports that triggered inspections. The most recurring issues involve infection control, medication management, and food safety, with problems appearing consistently across 2023-2025 surveys. While the facility reports correcting these deficiencies, the persistent recurrence of similar problems—particularly infection control appearing in all three survey periods—suggests ongoing operational challenges families should carefully consider.
