What this means for your family

This facility has a strong reputation for its physical therapy team, making it a potential option for short-term rehab. However, the recurring reports of neglect and slow response times are significant red flags; if you choose this facility, ensure you have a plan for frequent, unscheduled visits to monitor your loved one's care.

Key Review Excerpts

My mother was in a bad situation at another skilled nursing home after falling and breaking her arm. We were able to move her with the HUGE help from Director of Admissions Jessica Butker. She and the staff at Southwestern have been amazing to my mother and are helping her get back to her old self.

Long-term resident's family · 2025★★★★★

The nurses and staff have mostly been nice and dona good job. However, when you walk in the lobby and come to the building looks beautiful, when you get on the units they are not as beautiful. Not clean public restrooms is in great need of repair.

Family member · 2025★★★☆☆

Currently resident in rm 130 wheelchair left arm has been broken for over 4 days. Staff has been notified repeatedly. This is Medicare definition of 'neglect'. No communication or don't care.
