What this means for your family

While the facility has a strong rehab program and an engaging activities calendar, the reports of neglect and communication failures are serious. I strongly recommend visiting unannounced during off-hours or weekends to observe staffing levels and response times for yourself before making a decision.

Southern New Hampshire Rehabilitation & Healthcare presents a polarized experience, with some families praising the dedicated rehab therapy team and recent facility renovations, while others report severe neglect and understaffing. While some residents enjoy the improved food and active social calendar, multiple families have raised alarming concerns regarding hygiene, delayed response times, and poor communication during medical crises. Prospective families should be aware that experiences appear to vary significantly based on the specific unit and shift.

Questions for Your Tour

  • 1I noticed the facility has been undergoing renovations; how have these updates impacted the daily living environment and comfort for residents?
  • 2With a large capacity of 290 residents, what specific systems are in place to ensure that each resident receives consistent, timely assistance with personal care and hygiene?
  • 3Given the importance of clear communication, what is your standard process for keeping family members updated on changes in a loved one's medical status or health needs?
  • 4I understand the rehab therapy team is highly regarded here; how do you ensure that same level of dedicated support carries over into the nursing and medication management side of daily care?
  • 5What is the current approach to staffing during peak hours to ensure that residents' requests for assistance are addressed promptly?
  • 6Could you walk me through how the social activities and events are tailored to keep residents engaged, and how you encourage participation for those who might be more reserved?

Key Review Excerpts

This nursing home is clearly understaffed, and that’s something many facilities are struggling with right now. Because of this, care may not always be immediate, and small details like hair washing or clothing changes can take longer than anyone would like.

Long-term resident's family · 2025★★★★

My grandfather came here, was put in a temporary rehab room... It was fine. He was then moved up to a long-term rehab unit. I could talk about the lack of care, and the filth from this unit, but I’ll skip over that.

Long-term resident's family · 2025☆☆☆☆

Nurses do not have any knowledge. Ours was googling the medication she was on and the side effects she was having in front of us while demonstrating he had no idea what he was talking about.

2 complaint-triggered

This facility shows persistent problems across multiple areas, with families filing complaints about living environment conditions and foot care. The most recurring issues involve infection control, food service and dietary concerns, and fire safety violations that have repeated across all four surveys over the past two years. While all deficiencies show correction dates, the pattern of recurring violations in critical areas like infection prevention and food safety suggests ongoing operational challenges that families should carefully consider.
