Staff Nathan Rossbach mistreated Resident A by threatening to spray him with cold water, placing a frozen bag of corn on his back, and hitting him with a book.
Staff Shaun Hansen used inappropriate physical intervention on Resident C.

Resident A was given a discontinued dose of Prozac (an additional 10mg) on 7/19/2025 and 7/20/2025 because the discontinued medication card was not removed from the cart.
Resident A was not given his required dose of Linzess on 8/12/2025 due to staff failing to scan the medication.
