What this means for your family

This facility's strong rehab therapy team makes it a solid choice for post-surgery recovery, but it is not recommended for residents who require high levels of daily assistance or cannot advocate for themselves. Families should be prepared to visit frequently to ensure basic needs are met and should specifically ask about weekend staffing ratios and the process for escalating medical concerns to management.

Signature Healthcare at Jefferson Place is a facility that receives polarized feedback, with many families praising the beautiful, clean building and the effectiveness of the rehabilitation therapy team. However, a significant number of reviewers report serious concerns regarding neglect, poor communication, and inconsistent staffing levels, particularly during weekends and nights. Families considering this facility should weigh the positive experiences of rehab patients against the recurring reports of inadequate care for residents who cannot advocate for themselves.

Key Review Excerpts

Do not recommend this place if your loved one can’t be their own advocate. Many of the Caregivers were great (and I know meant well), but communication between them, the doctor and their Nursing Director and other leadership was terrible.

Long-term resident's family · 2022★★★☆☆

The CNA's are overworked. The nurses, with exception to one, were unwilling to help the patients. I was the patient, and on average, I was waiting 20 minutes or longer for a CNA to come in and help me.

Rehab patient · 2025☆☆☆☆

My father was discharged from Norton Brownsboro in March 2023 to Jefferson Place for rehab. Don't expect any new clinical issues to be dealt with unless it's Monday through Thursday. Also, you better expect to remind them to give your friend/family member/loved ones their meds on time or it will never happen.
