What this means for your family

While the therapy team is frequently praised for helping patients regain mobility, the facility suffers from significant inconsistencies in daily care and staffing. When touring, ask specifically about their call-light response protocols and how they handle meal service errors, as these are the most common complaints from families.

Signature Healthcare at Jefferson Manor receives highly polarized feedback, with many reviewers praising the dedicated nursing and therapy staff, while others report severe neglect and communication failures. While some families highlight a warm, family-like environment and successful rehab outcomes, multiple critical reports cite issues with hygiene, understaffing, and poor food quality. Families considering this facility should be aware of these inconsistent experiences and perform a thorough in-person visit.

Questions for Your Tour

  • 1I noticed your team is very active in responding to online feedback; how do you use that communication loop to address concerns and keep families informed about their loved one's care?
  • 2With a 3-star CMS staffing rating, what steps are you taking to ensure that residents receive timely assistance when they need help with daily tasks or call lights?
  • 3I understand that dining experiences can be challenging in a facility this size; could you walk me through how you handle meal service to ensure residents receive the correct, high-quality nutrition they need?
  • 4Given the recent focus on maintaining high hygiene standards, what is your daily protocol for ensuring residents receive consistent, personalized bathing and personal care support?
  • 5What does a typical day look like for residents here, and how do you tailor activities to keep them engaged given the facility's capacity of 100 residents?
  • 6How is your medical team structured to handle medication management and urgent health changes, especially during evening or weekend hours when staffing levels may fluctuate?

Key Review Excerpts

My Husband went in this place for Rehab and It turned out to be a nightmare. The place looks nice when you go to check it out but the staff was horrible and the feed the residents like dogs.

Rehab patient's spouse · 2025☆☆☆☆

The thing I love most about Jefferson Manor is that each time we visit there are at least 5 - 10 nurses, aides, other staff available for questions, assistance, etc.

Long-term resident's family · 2025☆☆☆☆

My mom waited hours after pushing her call light for multiple issues, including needing assistance to use the restroom. They consistently messed up her medicine schedule which kept her in pain.
