Key Review Excerpts

My grandmother stayed here for just one week, and it was one of the worst experiences we’ve ever had. During that time, she was not bathed or changed regularly—we had to go in ourselves to clean her and change her diaper because she was developing a rash from being neglected.

My boyfriend had a stroke and was taken there for rehab this place needs shut down. When we arrive the room was not set up it was filthy the whole place smelled of urine and feces no working TV The call light was trapped underneath the bed around the bed frame after we was settled no one would answer his call light.

Partner of rehab patient · 2025☆☆☆☆

Lyndon Crossing has a concerning pattern of repeated violations triggered by multiple family complaints, with 114 deficiencies across 9 surveys. The same critical issues persistently recur including care planning failures, safety hazards, and medication management problems. Families have filed numerous reports about abuse protection failures and inadequate nursing staff levels. While the facility reports correcting individual violations, the ongoing cycle of similar problems across multiple complaint investigations suggests systemic care quality issues that families should carefully consider.
