The facility underwent an Annual 1-year required inspection on October 20, 2025. The inspection covered various aspects of care, including resident rooms, safety equipment, and medication management. No deficiencies were cited during this visit, indicating full compliance with current licensing standards.

This report details a complaint investigation conducted on October 29, 2024, regarding allegations that staff interfered with resident visits. After reviewing records and interviewing multiple parties, the allegation was found to be unsubstantiated due to a lack of corroborating evidence and conflicting statements.

The unannounced Annual Required inspection found several areas needing attention, primarily related to documentation and staff compliance. Specific deficiencies noted include an outdated physician's report for one resident, multiple staff members lacking current first aid certification, and medication records requiring reconciliation in the memory care unit. The unannounced annual inspection conducted on 10/11/22 found no deficiencies. The LPA observed proper infection control practices, adequate supplies, and appropriate storage of medications and supplies. The facility was advised to submit updated licensing forms by 10/18/22.

This report details a follow-up case management visit concerning a serious incident that occurred on 5/14/2022. The investigation concluded that the incident was due to self-inflicted harm, and there was no preponderance evidence of neglect or lack of supervision from the facility. No deficiencies were cited during this visit.
