What this means for your family

We strongly recommend that you conduct an unannounced visit to check the cleanliness of the rooms and ask management directly about their current staffing ratios and protocols for night-shift supervision.

Key Review Excerpts

The facility is older but kept ruthlessly clean. We have been especially grateful to Terry Black (finance), Velma Wade (social svs.), nurses Billie, Kelly and David, the physical therapists, the gracious CNAs and support staff.

Long-term resident's family · 2026★★★★★

I have a family member here ,if I could move her I would . Plumbing backed up frequently, roaches .no area for the shut ins to go out side safely . Personal items go missing . I feel they are under staffed .

Family member · 2025☆☆☆☆

My mom stayed at Pruitt Health Holly Hill after being discharged from the hospital because she needed rehabilitation. The staff there treated her with patience, kindness, and professionalism.

This facility has ongoing compliance concerns with 34 deficiencies across three surveys, including four complaint-triggered issues where families reported problems. While all deficiencies show correction dates, the pattern of recurring problems in care planning and resident rights across multiple surveys suggests persistent operational challenges that families should investigate further during visits.
