Key Review Excerpts

The 1st Floor skilled level care, especially the recovery therapists, is very good. They are kind, caring, well staffed and did a pretty god job with our loved one. Since she seemed to do well on the skilled recovery floor, we decided to let Mom stay as a long term resident. We did not know what we were in for! Those floors look nothing like the photos.

Long-term resident's family · 2025☆☆☆☆

My sister was sent here from the hospital for rehabilitation. I always say you have to love your job when it comes to health care. My sister was calling me saying no one had checked on her and hadn't been changed. She was left for hours and no one came to her room to even check on her.

Rehab patient's family · 2023☆☆☆☆

The administrator (Joseph Dawson) and DON (Evelyn Freeman) are unprofessional and unresponsive to concerns. The administrator is not personable and does not seem willing to help when concerns are raised. Communication is poor, and issues are not addressed in a timely or caring manner.

This facility has a concerning pattern of recurring problems, with several families filing complaints triggering investigations over safety and care issues. The most frequent problem areas involve medication management, fire safety systems, and resident assessment procedures. While the facility has corrected all cited deficiencies, the persistent recurrence of medication errors and safety violations across multiple surveys suggests ongoing operational challenges that families should carefully evaluate before considering placement.
