Pleasant Hill Oasis

This was an unannounced Plan of Correction (POC) visit conducted on 10/02/2025. The facility was cited for three deficiencies, all of which are Type B violations. These deficiencies relate to ongoing maintenance issues, including unclean physical plant conditions and failure to provide updated resident reappraisals.

The inspection identified multiple deficiencies related to facility maintenance, recordkeeping, and resident health documentation. Specifically, issues were noted regarding cleanliness of common areas, window screens, and failure to keep required resident records and medical assessments current. All cited deficiencies were classified as Type B, indicating potential but not immediate health risks.
