Key Review Excerpts

The place could be cleaner. They could answer the phone regularly. They could help the patients a little bit with their nails and hair, which would make them feel better for sure. I noticed unchanged diapers, and they must be changed more.

Long-term resident's family · 2025★★☆☆☆

This place should not be in service, they never answer the phone so I got on a flight and showed up to find my family member tied up in his wheelchair I was shocked, no sheets on his bed ,I asked for wash cloth to wash him they said there is paper towels in bathroom, and there was none, please don't go here.

Families have filed complaints triggering inspections, including concerns about accident prevention and respiratory care. The facility shows recurring issues with resident assessment and care planning, medication management, and infection control across multiple surveys from 2022 to 2023. While all deficiencies have correction dates, the pattern of repeated problems in core care areas suggests ongoing quality challenges that families should investigate further during visits.
