Based on observation and interview, the manager failed to ensure poisonous or toxic materials stored by the facility were stored in a locked area and were inaccessible to residents. Findings include: 1. During a tour of the facility, the Compliance Officer observed an unlocked laundry room, which was accessible to residents. The Compliance officer observed a bottle of "Alkaline Floor Cleaner", a bottle of "Rapid Multi Surface Disinfectant Cleaner", a bottle of "WAXIE Kleen-Brite”, and a bottle of “Pledge” furniture polish unsecured on the counter. 2. The Compliance Officer observed a keypad lock on the handle of the laundry room door. E5 attempted to lock the door and the lock would not engage. E2 contacted maintenance and requested an immediate repair. 3. In an interview, E1 acknowledged the toxic materials were unlocked and accessible to residents.
