Based on record review and interview, the health care institution's chief administrative officer failed to ensure an individual for whom baseline screening and documentation of freedom from infectious tuberculosis (TB) was required, obtained as evidence of freedom from TB: c. If the individual had a positive Mantoux skin test or other tuberculosis screening test according to subsection (B)(1)(a) and does not have history of tuberculosis or documentation of latent tuberculosis infection, as defined in A.A.C. R9-6-1201, a written statement: i. That the individual was free from infectious tuberculosis, signed by a medical practitioner or local health agency, as defined in A.A.C. R9-6-101; and ii. Dated within 12 months before the date the individual began providing services at or on behalf of the health care institution, for one of two personnel records reviewed. Findings include: 1. A review of E3's personnel record revealed documentation of a positive T-SPOT\'ae TB Test collected on June 23, 2023. The documentation revealed the Medical Practitioner reviewed the test on June 26, 2023 and wrote, "Recommend QuantiFERON Gold Plus Test". 2. A review of E3's personnel record revealed documentation of a negative skin test result from North Carolina, which was dated May 10, 2023. 3. A review of the staff schedule revealed E3 was working in the facility at the date and time of the survey. 4. In an interview E5 reported E3 had been advised by the doctor to get a chest x-ray and E3 did not want to have an x-ray more than once every 5 years. E5 reported E3 scheduled an appointment with E3's primary care physician to be cleared. 5. In an interview E2 and E5 acknowledged the health care institution failed to obtain as evidence of freedom from TB, if an individual had a positive Mantoux skin test or other tuberculosis screening test and does not have history of tuberculosis or documentation of latent tuberculosis infection, a written statement that the individual was free from infectious tuberculosis, signed by a medical practitioner or local health agency, and dated within 12 months before the date the individual began providing services at or on behalf of the health care institution, for one of two personnel records reviewed. E5 reported E3 would be sent home immediately and until cleared.
