The facility failed to ensure that oxygen cylinders were stored securely in a rack or crate. Five unsecured O2 cylinders were found in a semi-private resident room, posing a risk of being bumped or displaced.

The facility failed to maintain hot water temperatures within the required range of 100 to 116 degrees Fahrenheit. Observations revealed water temperatures as high as 127.3 degrees F in a hallway shower and 126.3 degrees F in a semi-private bathroom sink.

The facility failed to maintain a hazard-free environment due to four unsecured oxygen cylinders sitting upright on the floor in a resident room. There was no rack or crate provided to secure the cylinders, and staff failed to report the lack of storage or request equipment from hospice services.

The facility failed to ensure the building met North Carolina State Building Code requirements for non-ambulatory residents. Specifically, one resident who was physically and/or cognitively impaired was unable to evacuate the facility independently without assistance, despite the facility being licensed for only six ambulatory residents.
