A relicensure survey was completed on 11/18/25. Deficiencies were cited. Based on interview and record review the residence failed to, on a quarterly basis, audit the accuracy and completeness of medication administration records (MARs), affecting eight current residents.Findings include:On 11/18/25 at 8:45 a.m. the last three quarterly medication audits were requested from the administrator. However, at 3:45 p.m. a document titled medication cart audits was provided. The medication cart audit revealed no evidence of medication administration records audits had been conducted. On 11/18/25 at 4:25 a.m. the administrator stated he was unaware of the requirement to perform and document medication administration audits. He stated he had no do.. Based on record review and interview, the residence failed to develop care plans that included input from the residents or their representatives, detail specific service needs along with the staff tasks necessary to meet those needs, or identify formal and informal engagement opportunities that match the residents personal choices and needs affecting four of four sample residents (#1-#4). Findings include:1. Residence PolicyThe residence policy dated 8/29/22 read in pertinent part that the residence will develop an individualized person-centered care plan and should address, but not be limited to, psychosocial needs, including activities, spiritual needs and any need identified by th.. Based on record review and interview, the residence failed to have an involuntary discharge grievance policy that included all required elements, affecting eight current residents.Findings include:A review of the residence' s discharge and grievance policies and the termination section of the resident agreement revealed that the residence did not include the following required parts of the involuntary discharge grievance policy: (1) The ability for the resident, or other person allowed to file a grievance, to receive assistance in preparing and filing a grievance without interference from the assisted living residence.
