A Licensure survey, prompted by #CO40237 was conducted on 6/16/25. A deficiency was cited. Based on observation, record review, and interview, the residence failed to ensure each room had sufficientventilation, and cooling to meet the needs of the residents affecting 20 current residents.Findings include:1. Reference and Resident Agreement According to Weather Underground (2025) on 6/16/25 at 11:54 a.m., the temperature was 86 degrees F and at2:54 p.m., the temperature was 90 degrees F. The Residency Agreement dated 1/25/25 for Resident #1 read in part that the residence would ensure thatresidents the care necessary to protect their health, safety, and well-being.2. ObservationsOn 6/16/25 from 7:59 a.m. until 3:17 p.m an environmental tour using a calibrated thermometer revealed:At 8:40 a.m., the room of Resident #1 was 76.5 degrees F. There was minimal air flow coming from the unit and itwas warm to the touch.At 8:30 a.m., the hallway temperature outside Resident #2' s room was 79 degrees F.At 10:06 a.m., the control panel accessing the outside main AC unit #5 was open and the unit was not operating.At 11:52 p.m., the room of Resident #2 was 76.6 degrees F. There was minimal air flow coming from the unit and itwas warm to the touch.At 12:25 p.m., the room of Resident #4 was 78.4 degrees F.At 1:00 p.m., the room of Resident #1 was 82.6 degrees F.At approximately 2:40 p.m. (the administrator and regional director indicated that they were monitoring the roomof Resident #1 and the initial temperature was 80.7 degrees F).3. Record reviewResident #1 was admitted to the residence on 1/3/25 and was later diagnosed with low oxygen saturation. A written practitioner' s order, dated 4/25/25, directed the residence to administer oxygen 2-4 L/min via nasalcannula.
