While the physician's visit note from 10/14/22 indicated a reduction of metoprolol to 12.5mg daily, the medication bubble pack and administration records still reflected the previous higher dose of 25mg every 12 hours.

The facility failed to ensure the physician's order for a dose reduction of metoprolol tartrate was verified and implemented. Although the physician's visit note from 10/14/22 indicated a dose reduction to 12.5mg, the resident continued to receive 25mg as documented in the eMAR and medication bubble pack through 12/07/22.
