Key Review Excerpts

The response time when you press the help button was horrible. I waited for a 1-1/2 hours one day. Most of the nuses were mean and acted like I was such a bother to them.

Rehab patient · 2021★★☆☆☆

My mom was admitted there for rehab after having a stroke. She enjoyed her stay. She liked the entertainment, staff and dedication of her providers getting her better to go back home.

Long-term resident's family · 2022★★★★★

The DON reached out and everything has been addressed and taken care of! I have also updated my review from 1 star to 3. All nurses will receive mandatory training on CPAP/BIPAP machines, they moved my mom to a new/clean room, dietary has addressed the food issues, rude staff will be dealt with and her GCM situation has been resolved.

5deficiencies
Well below state avg (10.1)
3 complaint-triggered

This facility shows recurring problems with fire safety, medical records, and medication management that persist across multiple years. The most concerning pattern involves medical record security violations, including three complaint-triggered deficiencies where families filed reports about accessing resident records. Fire safety issues appear repeatedly across 2019, 2022, and 2024 surveys, though the facility corrects each violation. While all deficiencies show correction dates, the repeated nature of these safety concerns warrants careful evaluation during any visit.
