What this means for your family

While the therapy and activities departments receive some praise, the facility is currently facing critical reports regarding basic infrastructure, such as lack of water, and severe medication management errors. We strongly advise families to look elsewhere, or if considering this facility, to conduct an unannounced visit and verify current state inspection reports regarding staffing and sanitation.

Key Review Excerpts

They left my dad choking in his room and the lights off. I showed up, walked by the nurses station. They stopped me and said that he aspirated during lunch didn’t call a doctor.

Family member · 2024☆☆☆☆

Upon entering the strong smell of urine and feces in the air was overwhelming. While there, I've talked to other residents who escorted me to their rooms to show me the disarray of clothes, all over the floor and broken furniture.

Visitor · 2025☆☆☆☆

My mom was here and the nurses were nice the facility is a bit under staffed we had some problems but they stepped up to the plate and we ironed them out.

Grande Oaks shows concerning patterns with nearly half (44 of 99) deficiencies triggered by family complaints, indicating ongoing resident and family dissatisfaction. The facility struggles repeatedly with medication management, environmental safety and cleanliness, and nursing care quality across multiple surveys from 2019 to 2024. While deficiencies are marked as corrected, the persistent recurrence of similar issues—particularly medication errors appearing in six separate complaint investigations—suggests underlying systemic problems that families should carefully consider.
