Clean linens were stored with miscellaneous items (wheelchairs, boxes, etc.) and soiled linens were stored in a general storage closet with garbage and housekeeping items, posing cross-contamination risks. Additionally, soiled clothing/shoes were found behind dryers, creating a fire hazard.
Meal census and food records from November 2025 to February 2026 were incomplete or missing entries, making it impossible to determine the type and amount of food served.
Dishwasher sanitization logs for January and February 2026 contained incomplete or blank entries regarding sanitization temperatures.
