The facility underwent an unannounced Annual Inspection on May 1, 2025. The inspection covered facility tours, review of resident and staff files, and observation of various areas. No deficiencies were issued during this inspection.

The facility underwent an unannounced Annual Continuation Inspection. The primary deficiency noted relates to the improper storage of hazardous materials, specifically paint cans, which were accessible to residents in care. Immediate corrective action was required for this safety hazard.

The inspection was conducted on 5/15/2024 and covered an annual review of the facility. The LPA reviewed resident and staff files, observed the facility areas including dining room, kitchen, and courtyards, and noted that the facility has sufficient supplies for food. While the facility addressed the missing funds incident appropriately, a deficiency was cited concerning medication administration errors for Resident (R2). The primary deficiency found relates to the failure to assist residents with self-administered medications, posing an immediate health risk.

The Licensing Program Analyst reviewed the facility records and observed that very few incident reports were available from October to December 2023. No deficiencies were noted during the visit.
