What this means for your family

This facility has a strong reputation for rehabilitation and a warm, family-like environment, especially under recent management. However, families must be aware of recurring reports regarding slow response times and inconsistent hygiene. We recommend scheduling unannounced visits and asking the administration directly about their protocols for call-light response and resident monitoring.

Key Review Excerpts

My husband was here for therapy after an accident to learn how to walk again. Nursing staff really slow to answer call lights. In fact he had to use his cell phone to call the main number to get assistance to go to the bathroom after waiting 45 minutes for someone to answer the call light.

Spouse of rehab patient · 2019☆☆☆☆

My dad was here for rehab after a surgery. The staff were incredibly competent and kind. Within 2 weeks the therapist had him walking normally. The nurses and techs were attentive to his needs and kind to family.

Family member of rehab patient · 2024★★★★★

They left my mother with 2 broken hips laying in Colitis Infection Screaming for HELP from 9 am till almost 5 pm And I had to Complain to get her changed then.

This facility has concerning patterns with 52 deficiencies across recent surveys, including families filing complaints that triggered inspections. The most frequent problems involve fire safety systems, electrical equipment maintenance, and nursing staff adequacy. While the facility reports correcting these issues, the recurring nature of fire safety and electrical deficiencies—appearing across multiple surveys from 2019 to 2025—suggests ongoing operational challenges that families should carefully evaluate before placement.
