Specifically, discrepancies were found between the eMAR and controlled substance logs for two residents, including instances where medication was documented as administered on the eMAR but not recorded on the controlled substance log, and instances where medication was administered more frequently than ordered.

Specifically, a PCA/MA prepared tea for a resident requiring honey-thick consistency using incorrect measurements of Thick-it powder, resulting in a liquid that was not at the required thickness.

The facility failed to ensure necessary follow-up appointments were scheduled for residents returning from the hospital. Specifically, there was an oversight in scheduling appointments for residents, such as Resident 6-3, who required a referral from the PCP for a neurologist. The facility has since implemented a plan of correction to ensure staff make calls to schedule overlooked appointments and that the Administrator checks records for residents returning from the ER.

The facility failed to maintain the kitchen freezer in a safe and operating condition to keep food frozen solid. Observations revealed freezer temperatures as high as 16 degrees F and items such as bologna and hotdogs were not completely frozen. Staff interviews indicated the freezer does not get cold properly and takes an excessive amount of time to freeze ice.
