The unannounced required - 1 year inspection visit was conducted on 10/08/2024. The facility was observed to be clean, with proper safety measures in place, and no citations were issued during this visit. The administrator was requested to submit several updated forms by 10/15/2024.

The facility underwent an unannounced Case Management visit on February 6, 2024. The purpose of the visit was to deliver an exclusion letter regarding a staff member. The report notes that the excluded staff member has not contacted the facility or been on the schedule since September 2023.
