COMPLAINT INVESTIGATION REPORT

This report details an investigation into allegations of resident pressure injuries and unexplained injuries. The investigation concluded that the complaint was unfounded, meaning the allegations were found to be false or without reasonable basis.

The facility underwent a required one-year annual inspection. The Licensing Program Analyst observed that the physical plant, resident rooms, kitchen, and storage areas were clean and in good repair. The report indicates no deficiencies were found during the visit.
