What this means for your family

While the facility offers strong rehabilitation services and engaging activities, the recurring reports of medication errors and hygiene issues are significant red flags. If you are considering this facility, we strongly recommend requesting a tour of the specific unit where your loved one will reside and asking pointed questions about their ratio of permanent staff versus agency staff.

Medina Center for Rehabilitation and Nursing receives highly polarized feedback, with some families praising the facility as a welcoming, clean environment with attentive staff, while others report severe neglect and safety concerns. Questions for Your Tour

  • 1I noticed the facility is decorated beautifully; how do you maintain that welcoming environment while ensuring the high standards of cleanliness and hygiene that are so important for resident health?
  • 2Given the importance of medication management, what specific protocols and double-check systems do you have in place to ensure residents receive the correct doses on time?
  • 3I see that you are active in responding to feedback online; how do you use that communication channel to address family concerns and keep us updated on our loved one's care?
  • 4With your focus on physical and occupational therapy, how do you ensure that residents receive consistent care when there is a reliance on agency or temporary staff?
  • 5I noticed the smoking area near the front entrance; is there a plan to relocate this to ensure a more pleasant and smoke-free environment for residents and visitors entering the building?
  • 6Beyond the structured therapy sessions, what does a typical day look like for residents in terms of social engagement and daily activities?

Key Review Excerpts

My father was there for about a month. Some of the staff was very nice and we really liked the physical therapy department. However, I was not pleased with his overall care. There was a huge lack of communication and everyone is a temporary.

Long-term resident's family · 2022☆☆☆☆

This place is horrible and should be shut down. My father was here after he had surgery. They constantly messed up his medications, missing doses giving the meds at the wrong times. Never showered him until we demanded one.

Rehab patient's family · 2024☆☆☆☆

I can't say enough about the Medina Center for Rehab and Nursing!!! My brother Dave has lived there for 1-1/2 years now and he is thriving! He receives excellent care from your amazing staff and we enjoy the picnics and fun activities for family too!
