The facility underwent an unannounced Case Management health and safety check on November 7, 2025. The LPA observed that lighting was adequate, bathrooms were equipped with necessary safety features, and there were sufficient supplies of food and medications were centrally stored and secured. No deficiencies were cited during this visit.
