No single service towels were available for handwashing in the memory care kitchenette.
Dishwasher sanitization logs contained incomplete or blank entries regarding sanitization temperatures.
Clean linen closet in the assisted living laundry area contained miscellaneous boxes and a clothing hamper, risking cross-contamination.
Meal census and food records for September 2025 to December 2025 were incomplete and/or missing entries.
Housekeeping items (broom and dustpan) were stored in the dry food storage area instead of the janitor's closet.
