Based on the review of facility records, staff interviews and observation the facility failed to ensure that physical restraints were only used according to a physician's written order and with the written consent of the resident or his legal representative. Evidence: Resident #2 The inspector accompanied by the facility Administrator observed that, resident #2 was in a Geri chair without a written physician's order. During interview facility staff stated that the Geri chair is being used due to the resident's difficulty sitting upright but did not obtain a written physician's order to implement the task.

An initial onsite inspection was conducted at the facility on 02/28/2022. No residents were in care. A walk through of the physical plant was conducted accompanied by the Licensee and the Facility Administrator. Measurements were conducted in four of the five bedrooms. The inspector did not observe any obvious concerns regarding the interior or exterior of the physical plant of the building. Mock files for residents and staff as well as other facility documentation and policies and procedures were reviewed for compliance during this initial application process. Technical assistance was offered. Information gathered during the inspection determined that staff credentials needed to be verified regarding staffing and personnel.
