The facility failed to ensure snacks were served to all residents as required by regulation. Observations and interviews revealed that residents were not being served snacks during the first shift, and some residents reported not receiving snacks for several days.

The facility failed to administer medications and treatments in accordance with physician orders for one resident. Specifically, the resident was not provided with portable oxygen as ordered, and there was no documentation in the electronic medication record to show that prescribed oxygen was administered.

The facility failed to ensure that snacks were offered or made available to all residents between meals. Observations and interviews revealed that residents were not being served snacks during the first shift, and staff members were unsure of responsibility for snack distribution.

The facility failed to ensure that the preparation and administration of medications and treatments by staff were in accordance with orders by a licensed prescribing practitioner and facility policies.

The facility failed to develop and implement an activity program that promotes active involvement of residents with each other, their families, and the community. Observations and interviews revealed that scheduled activities, such as 'Spa Day' and 'Wii Boxing,' were often not conducted as planned, and the activity calendar lacked end times. Additionally, staff responsibilities for transportation often conflicted with the ability to facilitate scheduled programming.

The facility failed to develop and implement an activity program that promotes active involvement of residents with each other, families, and the community. Observations and interviews revealed that scheduled activities, such as 'Spa Day,' 'Movie and Popcorn,' and 'Wii Boxing,' were often not conducted as scheduled or were held in different locations than intended. Staff interviews confirmed that activities were frequently missed or delayed due to the Transporter/Activities Director being away on medical appointments.
