The facility failed to ensure that one of six sampled staff members had no substantiated findings listed on the North Carolina Health Care Personnel Registry. Specifically, the personnel file for a Medication Aide/Supervisor-in-Charge lacked documentation of a required HCPR check. The Business Office Manager stated the original check may have been misplaced during an office move.
