What this means for your family

While this facility shows success in short-term physical therapy, the recurring reports of neglect and slow response times are significant red flags. If you choose this facility, ensure you have a plan for frequent, unannounced visits to monitor hygiene and care, and verify that their staffing levels are adequate for your loved one's specific medical needs.

Questions for Your Tour

  • 1I noticed you have a high response rate to online reviews; how do you use that feedback loop to improve communication with families regarding a loved one's daily medical status?
  • 2Given the focus on rehabilitation here, what specific protocols are in place to ensure consistent hygiene and personal care assistance for residents who are not in the short-term rehab program?
  • 3With the facility's current staffing rating, how do you manage call light response times to ensure residents receive timely assistance throughout the day and night?
  • 4Could you walk me through the specialized training or support systems provided for staff members who work directly with residents experiencing cognitive decline or memory challenges?
  • 5Since medication management is a priority, what steps do you take to ensure accuracy and clear communication with families when there are changes to a resident's care plan?
  • 6What does a typical day look like for residents in terms of social engagement and activities, and how do you ensure those with varying levels of mobility are included?

Key Review Excerpts

My 84 year old mother broke all 3 bones in her ankle, and these folks played an integral part in getting her back on her feet. They took excellent care of her through the first part of her rehab.

Rehab patient's family · 2024★★★★★

If you have a family member with dementia, or any significant cognitive difficulties, I do not recommend this facility for them. They accepted our family member with late-stage dementia... and quickly into his stay they called a family care conference to discuss moving him to a more 'appropriate facility'.

Memory care family member · 2026☆☆☆☆

My father was there recently and it was the worst care center I’ve ever seen... 1. Had him in a diaper when he could use the bathroom with a little help... 2. They only gave him a shower on Tuesdays and Thursdays.

This facility has a concerning pattern of families filing complaints that triggered 25 deficiency reports across 18 inspections. The most recurring care issues involve abuse and neglect protection, accident prevention and safety, and infection control. While all deficiencies show correction dates, the persistent pattern of complaint-driven investigations—including multiple recent cases involving abuse reporting failures and safety hazards—suggests ongoing care quality concerns that families should carefully evaluate during visits.
