A relicensure survey was completed on 10/23/24. Deficiencies were cited. Based on record review and interview, the residence failed to accurately document each medication administration at the time the event was completed on the medication administration record (MAR), affecting three (#1-#3) of three sample residents. (Cross-reference S1604)Findings include:1. Resident #2 was admitted to the residence on 7/26/24. Ciprofloxacin A written practitioner ' s orders, dated 10/10/24, directed the residence to administer ciprofloxacin 500 mg, one tablet by mouth twice daily for 14 days. However, the October 2024 MAR, contained blank space on 10/16/24. Similar deficient practice was found for aspirin, omeprazole, sertraline, tylenol and metronidazole. 2. InterviewsOn 10/23/24 at 11:10 a.m., the director of wellness acknowledged the blank spaces on September and October MARs for Resident #2 meant the medications were not administered. She also acknowledged the blank spaces on October MARs for Residents #1 and #3.On 10/23/24 at 12:05 p.m., the Administrator acknowledged the residence .. Based on record review and interview, the residence failed to ensure the administrator and qualified medication administration persons supervisor on a quarterly basis, audited the accuracy and completeness of the medication administration records, affecting 14 current residents. (Cross-reference S1600)Findings include:On 10/23/24 at 8:30 a.m., the last two quarterly medication audits were requested from the director of wellness. However, no documentation was provided.On 10/23/24 at 8:55 a.m., the director of wellness stated the residence had not completed or documented quarterly medication administration record audits. The director of wellness stated she and the resident care coordinator performed weekly medication cart audits and documented medications that needed to be ordered and documented medication errors. On 10/23/24 at 11:27 a.m., the administrator stated she was aware of the requirements to perform and document quarterly medication administration audits. She acknowledged the reside.. Based on record review and interview, the residence failed to have at least one staff member on-site at all times who had current certification in first aid from a nationally recognized organization, affecting 14 current residents.Findings include:A review of staff first aid certifications on 10/22/24 revealed that Staff #4-#6 and the residence house manager did not have first aid certification from a nationally recognized organization. The staff schedule revealed the following shifts did not have a person onsite at all times certified in first aid from a nationally recognized organization during the following shifts: The first shift (6:00 a.m. until 6:00 p.m.) on 10/2-10/5, 10/9-10/12, and 10/16-10/19/24. The second shift (6:00 p.m. until 6:00 a.m) on 10/13 and 10/14/24.On 10/22/24 at 12:55 p.m., the administrator and assistant administrator both said they knew they were required to have at least one first aid certified staff member in the residence at all times.
