The facility failed to ensure all required paperwork was completed for employees upon hire or transfer. Additionally, there was a lack of quarterly audits to ensure employee records remained up to date and that medication training and job descriptions were properly updated during staff transfers.
The facility failed to ensure that all food items stored in the refrigerator were properly labeled. There was no consistent process in place to check that opened containers were correctly identified.
