The resident admitted to hiding these medications from staff to avoid detection of his unauthorized self-administration.

The administrator failed to ensure the monitors were positioned to prevent unauthorized exposure of resident activities.

The facility failed to ensure the privacy and dignity of residents by placing video/audio monitors on a kitchen countertop facing the common area. This setup allowed visitors and other residents to observe private care activities, such as staff changing briefs and dressing residents, through the active monitors.

The facility failed to ensure unlicensed staff were competency validated by a registered nurse for performing in and out urinary catheterizations. Specifically, for one resident requiring catheterizations twice daily, the facility's contracted nurse had not trained or validated staff competency for this task.
