The facility failed to ensure that one of four sampled staff members had a completed criminal background check in their personnel file. Specifically, a medication aide was working and administering medications without documented proof of a background check performed upon hire.

The facility failed to ensure that a resident's medical examination was completed no more than 90 days prior to admission. A review of the resident's FL-2 form showed the medical examination did not meet the required timeframe.
