There is a follow-up letter dated 12/29/2025 stating these specific deficiencies were corrected.; Page 10 of 10. The Administrator signed the Plan/Attestation Statement on 12/2/25, with a target compliance date of 12/14/25.
Facility failed to verify nurse delegation core training for 3 staff and failed to re-evaluate delegation for 1 resident every 90 days. Also failed to obtain written consent for delegation.
Facility failed to ensure character, competence, and suitability (CCS) determination was completed for staff member with 'reviewed required' background check result.
Facility failed to complete a full assessment within 14 days of admission for 4 of 4 sampled residents.
Facility failed to complete TB testing within 3 days of hire for 3 of 4 sampled staff.
Staff D was hired on 09/23/2025 and Staff E was hired on 07/30/2025. No documentation of TB tests completed within 3 days of hire was found for either staff member. Staff A acknowledged that TB tests for Staff C, D, and E were not completed per regulation.
