Specifically, residents requiring pureed or mechanical soft diets were served food with incorrect consistencies, such as non-pureed oatmeal and ground pork loin that had not been processed to a whipped consistency.

The Resident Care Director was aware of the requirement but could not locate the necessary documentation and had not contacted the resident's physician for guidance.
