The facility failed to ensure proper health care referral and follow-up for a resident requiring diabetic shoes. Despite a physician's order for diabetic shoes for diabetic neuropathy, the facility did not follow up with the podiatrist's office or the medical supply company to ensure the resident received the necessary footwear.

The facility failed to ensure that one of three sampled residents had a discharge summary signed by a prescribing provider or a completed FL-2 upon returning to the facility from a 9-day hospitalization. Specifically, Resident #1 returned with a discharge summary that lacked the required physician signature.

The facility failed to assure the health care needs of residents, specifically regarding the coordination of referral and follow-up care. This was evidenced by the failure to obtain necessary medical orders/documentation for a resident following a hospital discharge.

The facility failed to ensure that one of three sampled residents had a completed FL-2, transfer form, or a discharge summary signed by a prescribing practitioner upon returning from a hospital stay. Specifically, Resident #1 returned from a 9-day hospitalization with a discharge summary that lacked the required physician signature.
