COMPLAINT INVESTIGATION REPORT
This report details the findings of a complaint investigation conducted on 11/07/2025 regarding allegations of neglect and failure to ensure resident needs were met. Based on the review of records, the department determined that there was not a preponderance of evidence to prove the alleged violations occurred. Consequently, all allegations were deemed UNSUBSTANTIATED.
The facility underwent an unannounced Case Management Visit conducted by LPA Ramon Serrano. The visit was to discuss an amended complaint report that was originally delivered on September 11, 2025. No deficiencies were cited or observed during this inspection.
