What this means for your family

This facility offers an exceptional physical therapy program and a beautiful, modern environment that is excellent for short-term rehabilitation. However, families must be extremely vigilant regarding nursing care, as there is a documented pattern of delayed responses to call lights and serious hygiene neglect. If you choose this facility, we recommend frequent, unannounced visits to ensure your loved one's basic needs are being met.

Houston Transitional Care is frequently praised for its modern, hotel-like facility and its highly skilled physical and occupational therapy teams. However, families should be extremely cautious regarding reports of serious neglect, including failure to respond to call lights, improper catheter care, and patients being left in soiled bedding. While many reviewers highlight a warm and professional atmosphere, there is a significant pattern of concerns regarding understaffing and inconsistent nursing care.

Key Review Excerpts

The Rehab people are nice and so are some nursing and CNA staff. Food is tolerable but its presentation sometimes is less than appealing. Now the concerns: Staff don’t really talk to or engage the patient much just do their highly jobs quietly and leave.

Long-term resident's family · 2026☆☆☆☆

I was in a lot of pain but they would never give you medication in time. When asked, they said they legally had +/- 2 hours to give medication. Expect to be in great pain if you choose to go there.

Rehab patient · 2023☆☆☆☆

At my stay here at Houston Transitional Care it was very pleasant. With that I would would like to acknowledge the Activities Director Chelsey. She is a very energetic and positive Activities Director.

Houston Transitional Care has ongoing challenges across multiple areas, with 20 deficiencies found in three surveys from 2022-2024. The most frequent issues involve medication management, food service and sanitation, and care planning, with several problems recurring across different survey periods. One family complaint triggered an investigation regarding medication services. While the facility has corrected each cited deficiency, the pattern of repeated violations in core care areas suggests families should inquire about recent quality improvements.
