The facility failed to follow infection control measures during medication administration for several residents. Specifically, a Novolog FlexPen for Resident #8 was not labeled with the required opened-on date, posing a risk for improper medication management.

During breakfast observations, 27 of 27 residents were served meals without knives. Staff interviews indicated that knives were withheld from residents due to safety concerns regarding potential stabbing.
