The facility failed to follow infection control measures during medication administration for several residents. Specifically, a Novolog FlexPen for Resident #8 was not labeled with the required opened-on date, posing a risk for improper medication management.

The facility failed to provide a complete table service setting including a knife, fork, and spoon for all 27 residents. During breakfast service, residents were provided forks and spoons but no knives were included in their place settings.

The facility failed to follow the maintained kitchen menus to ensure residents received well-balanced meals. On 12/14/16, the lunch served (fried chicken strips, pinto beans, macaroni and cheese, cake, and iced tea) did not match the scheduled menu of Veal Parmesan, noodles, vegetables, and oranges.

The facility failed to provide a complete table service setting, which must include a knife, fork, spoon, plate, and beverage container. During breakfast observations, 27 of 27 residents were served meals without knives. Staff interviews indicated that knives were withheld from residents due to safety concerns regarding potential stabbing.
