Number of residents present at the facility at the beginning of the inspection: 41 The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. Number of resident records reviewed: 4 Number of staff records reviewed: 3 Number of interviews conducted with staff: 3 Observations by licensing inspector: Inspector reviewed 4 resident records and 3 staff records. Observed a medication administration pass along with the pharmacy review and medication management plan. Reviewed health care oversight, infection control policy, six month practice plan of resident emergencies, fire and health inspection and review of emergency preparedness and response. Observed dietitian review and liability insurance. Additional Comments/Discussion: An exit meeting will be conducted to review the inspection findings. The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law. Based on a review of documentation and interviews, it was determined that the facility did not ensure all staff shall be considerate and respectful of the rights, dignity, and sensitivities of persons who are aged, infirm, or disabled. Evidence: 1. On 8/16/25 during an altercation between resident #1 and staff #3, staff #2 and staff #3 began to record the incident with their cell phones. 2. Staff #1 confirmed that both staff #2 and staff #3 recorded the resident-to-staff staff altercation. 3. Staff #1 also confirmed that staff #2 and staff #3 did not follow facility policy in reference to prohibiting the use of photographic and audio devices. 4. It was confirmed by Staff #1 that staff #2 and staff #3 both received a written disciplinary action and were both taken off of the Memory Care Unit. 5. Staff #2 and staff #3 were also required to retake training on Resident Rights and Mandated Reporter training. ;
