My mom enjoyed living here until her care increased and there is a HUGE lack of staffing (though I was assured they could accommodate her needs). She often would not receive a response to her call button for 30-60 minutes (often never!), staff was frustrated and rude when called upon, medications were administered incorrectly.

My grandfather lived here for over a year and during that time it was nothing but issues and neglect. His toe nails were so long that they hurt him in his shoes and he couldn't put on underwear. His fingernails were also falling out.

Sunrise is a small environment with a big loving heart for caring for your loved one. We chose Sunrise for my 91 year old father. He was in a hospice situation; the care takers there cared for him in a very loving gentle way.

"12.2.2 Infection Control Officer: Each facility shall assign at least one (1) staff member responsible for the site management of the facility' s Infection Prevention and Control Program and training. This individual shall be responsible for the following: (2) Completing a minimum of 1.5 hours of continuing education in infection prevention and control on an annual basis from a nationally-recognized provider or the Department' s training program sufficient .. A relicensure survey with complaints #CO31584, CO31693, and CO34801 was completed on 10/29/24. Deficiencies were cited. Based on interview and record review, the residence failed to ensure that qualified medication administration persons (QMAP) did not administer medications pro re nata (PRN) or "as needed" to residents who were not capable of requesting the medication, affecting two of two sample residents (#4, #5) who resided in the secure environment. Findings include: Resident #4 and #5 were admitted to the secure environment on 6/5/24 and 7/23/23, respectively... Based on interview and record review, the residence failed to investigate allegation of abuse of residents and document the investigation process, affecting one of five sample residents (#5).Findings include: Resident #5 was admitted to the secure environment of the residence on 7/23/23. A progress note, dated 9/9/24, read in part that Resident #5 hit another resident.There was no other documentation in Resident #5' s record that indicated an investig.. Based on observation and interview, the residence failed to ensure residents had the right to full use of the assisted living residence common areas in compliance affecting 51 current residents.Findings include:1. Residence PolicyThe residence' s resident rights policy, dated September 2023 read in part that the residents had the right to the full use of the residence common areas in compliance with the house rules.2. ObservationsOn 10/29/24 from 7:00 a.m. to 5:30 .. Based on observation, record review, and interview, the residence failed to have a list of all staff who have current certification in first aid or cardiopulmonary resuscitation (CPR) up to date and available to all staff at all times affecting 51 current residents.Findings include:An environmental tour of the residence on 10/29/24 revealed no visible list of staff who were certified in CPR and first aid. On 10/29/24 at 7:20 a.m., Staff #1 stated that she was not awar.. Based on observations and interviews, the residence failed to provide the maximum degree of benefit from available residence services. The residence failed to ensure the fall prevention system, consisting of pull cords and pendants, were in working condition at all times, affecting 51 current residents. Findings include:1. ObservationsOn 10/29/24 at 1:21 p.m., Resident #7 pressed her pendant to call for assistance. After 20 minutes, no staff arrived to assist. On 10/..
