Resident A and C did not have health care appraisals completed within the 90 days prior to admission. Resident B did not have an annual health care appraisal completed for 2023.
Failure to conduct required quarterly fire drills: No drills conducted in 4th quarter 2022; no day or sleep drills in 1st quarter 2023; no sleep or evening drills in 2nd quarter 2023; no evening or sleep drills in 3rd quarter 2023; no evening or sleep drills in 4th quarter 2023. Additionally, 2024 drills were improperly combined with adjoining buildings and residents were not being evacuated as required.
